How can I access the Student Course Feedback Form?
You can access the form on or off campus from a desktop computer, laptop, tablet or smartphone. You will have access to the form from October 31st, 2016 to December 5th, 2016. 
What is the process for completing the form?
You should complete a form for each course that you took this semester. 
Log into your SONIS account and follow the link to the feedback form that is set up in Google Apps. 
Follow the instructions for completing each section of the form and remember to click on the Submit button on completion of the form. 
Are my responses confidential? 
Yes! Identifying information on students who complete the form is not attached to the responses, therefore your responses cannot be traced back to you. 
Will I be allowed to complete more than one form for each of my course sections?
No, you are limited to one evaluation per course section in which you are enrolled.   Once you submit the completed form for a course section, you will not be allowed to complete another form for that course section. If you attempt to complete another form, you will receive a message that indicates that you have already completed the form for that course section.
What should I do if I am being asked to provide feedback on a course or course section in which I was not enrolled?
If you are asked to complete a form for a course or course section in which you were not enrolled, you should not complete a form for that course. Report the inclusion of this course or course section to your Dean/Co-ordinator or Head of Department. 
What do I do if I do not see all of the courses in which I am currently enrolled? 
If you do not see a form for a course in which you are currently enrolled, you should report the matter to your Dean/Co-ordinator or Head of Department. 
Can I still complete the Student Feedback Form after the deadline?
No, you will not have access to the form after the deadline. Therefore you should provide your feedback on all the courses you took this semester before the deadline.
Do I have to provide feedback on my courses every semester?
Yes. Your courses and/or your lecturers change each semester, therefore you should provide feedback on your teaching and learning experiences each semester. 
Will I be provided with information on the results of the feedback exercise?
You will be provided with a brief report on the overall results following the review of the end of semester student assessment results by the Academic Board. 
How will the results of the feedback exercise be used?
The results will be used to inform decisions related to the provision of professional development for lecturers and improvement of the teaching and learning experience.
